CLARE Project

Role: To look out for older and vulnerable people within the community and helping to keep them safe and well. Tackle isolation and lonliness with lunch groups and day centres.

Connected Community Care

Role: Co-ordinates GP Referrals to link workers who meet up with the person referred to Social Prescribing and draw up a jointly agreed programme to meet that person’s needs

Role: Supporting families in North Belfast, providing Nutrition and Exercise Programmes, Social Media Awareness, Stress Management Clinics , Young peoples programmes etc

connected community care for cancer

Role: The Macmillan Community Link Worker supports individuals affected by cancer. They perform a Holistic Needs Assessment with the client, and the jointly come up with an invididualised plan which enables the person to access the support they need in their local community. This could be self-management programmes, diet and exercise advice, social or emotional support, smoking cessation or financial advice.
